About Chronically Informed

“Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both.”

More than 50 percent of the people in the United States live with a chronic illness, and more than 27 percent have multiple chronic conditions.*

Informing Our Community is what drives us.

Chronically Informed was created in November 2023 as Jessie Shay’s (she/her) Social Work Doctoral Capstone Project. Accessibility is extremely important to us. If you have any suggestions on how to make the platform more accessible, please reach out to us at info@chronicallyinformed.org

 

Chronically Informed is a project of the Chronic Care Collaborative. As a project under the umbrella of the Chronic Care Collaborative's work, Chronically Informed has  501(c)(3) status and operates as a nonprofit. The Archer Group/Fund for a Healthier Colorado is our fiscal sponsor.
