About Chronically Informed

“Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both.”

More than 50 percent of the people in the United States live with a chronic illness, and more than 27 percent have multiple chronic conditions.*

Informing Our Community is what drives us.

Chronically Informed is a nonprofit with 501(c)(3) status. We are fiscally sponsored by The Archer Group dba Fund for a Healthier Colorado.

Chronically Informed was created in November 2023 as Jessie Shay’s (she/her) Social Work Doctoral Capstone Project. Accessibility is extremely important to us. If you have any suggestions on how to make the platform more accessible, please reach out to us at info@chronicallyinformed.org.

Chronically Informed complies with applicable federal, state, and local civil rights laws and does not exclude, deny benefits to, treat less favorably, or otherwise discriminate against any individual on the basis of race, color, ethnic or national origin, documentation status, size, disability, ancestry, age, sex, gender, sexual orientation, gender identity and expression, intersex traits, religion, creed, political beliefs, pregnancy, military status, or marital status in employment, admission or access to, participation in, or receipt of the services and benefits under any of its programs, services and activities, whether carried out by the organization directly or through any entity with which it arranges to carry out its programs, services and activities. Chronically Informed does not intimidate or retaliate against any individual or group because they have exercised their rights to participate in actions protected, or oppose action prohibited, by 40 C.F.R. Parts 5 and 7, or for the purpose of interfering with such rights.
